Rafael Ortiz, 72, of Providence, Rhode Island, passed away peacefully at Kent Hospital in Warwick on September 3, 2026. Born in San Juan, Puerto Rico, on September 12, 1953, he was the beloved son of the late Felicita and Jose Gilberto Ortiz.

Known to all as a gentle, sweet, and caring soul, Rafael touched many lives with his quiet humility. He was a steadfast and unwavering presence for those he loved, always ready to brighten the days of those around him with his incredible warmth and great sense of humor.

Rafael’s strong faith in God was the cornerstone of his life. He deeply loved the Lord and his church, Iglesia Pentecostal Monte Sinaí, living out his beliefs through active compassion. He dedicated much of his time to volunteering at local soup kitchens and found profound purpose in serving as a prison chaplain, offering hope, kindness, and spiritual guidance to those who needed it most.

Above all else, Rafael cherished his family. He was a devoted father to Rafael Ortiz Jr. and his wife Noralis, his daughter Sara Peasley and her husband Ross, his daughter Coraly Mosher, his son Radames Berrios, and his son Jose Berrios. His legacy of love continues to grow through his 11 beloved grandchildren and 7 great-grandchildren, all of whom brought him immense pride.

He is also survived by five sisters, Carmen Ortiz, Norma Ortiz, Iris Ortiz, Olga Ortiz, and Felicia Arias; two brothers, Bertin Ortiz and Luis Ortiz; and many beloved nieces and nephews. He was predeceased by his brother, Jose Ortiz.
